  • PPP VAT Form – Preliminary VAT Module;
  • Form Annex 1 – Data on the base, i.e. fee and VAT from the electronic invoice system;
  • Form Annex 2 – Trade in equipment and facilities for performing activities and investments in facilities.

 

As a reminder, the PPP VAT Form is prepared on the basis of the data that are automatically entered into the Form Annex 1 and the data that the VAT payer has independently entered into the Form Annex 2.
